Transparency Report — Inaugural Edition

Reporting Period Covered: Pre-launch / Framework

I. Why We Publish a Transparency Report

Direct-to-consumer telehealth has scaled fast and the public has reasonable questions about what really happens behind a website. Teledris Health is committing to publishing a Transparency Report on a regular cadence so that patients, clinicians, regulators, and the public can hold us accountable to consistent, measurable practices.

This is the inaugural edition. Because Teledris Health is at or near launch, we do not yet have meaningful operating data to report. Instead, this edition publishes:

  1. The framework of metrics we will report.
  2. The definitions that will govern those metrics.
  3. The partners and structure behind our care today.
  4. A commitment to publish quantitative data on a defined schedule.

II. Reporting Cadence

Beginning with our second edition, we commit to publishing Transparency Reports on a semi-annual basis covering January to June and July to December reporting periods. Each report will be published within 60 days after the end of the reporting period.

III. Framework of Metrics

The following metrics will appear in every Transparency Report. Where a metric is not yet meaningful, the metric will be marked deferred with a note explaining when it will be added.

A. Volume and Decision Outcomes

  • Total intakes received in the period.
  • Percentage of intakes for which a prescription was issued.
  • Percentage of intakes declined for clinical reasons.
  • Percentage of intakes declined for eligibility reasons, including state, age, and identity verification.
  • Percentage of intakes withdrawn by the patient before review.
  • Median and 90th-percentile time from intake submission to clinician decision.

B. Clinical Profile

  • Distribution of medications prescribed, including compounded semaglutide, compounded tirzepatide, and other medications.
  • Distribution of starting doses.
  • Median duration of patients on therapy at the close of the period.
  • Reasons for discontinuation in aggregate categories such as side effects, insufficient response, achieved goal, cost, lifestyle, lost to follow-up, and clinically discontinued.

C. Geography

  • State-level distribution of patients, including counts and percentages.
  • Number of states in which Teledris Health is currently operating.

D. Demographics (De-identified, Aggregate)

  • Age distribution.
  • Sex / gender distribution as collected.
  • Language preference distribution, including English and Spanish.

E. Safety and Adverse Events

  • Aggregate number of adverse events reported through patient-initiated channels, by severity category.
  • Number of FDA Adverse Event Reporting System submissions made by Teledris Health partners on behalf of patients during the period, as available.
  • Number of internal Serious Adverse Event reviews opened and closed.

F. Patient Experience and Complaints

  • Total grievances received, by category.
  • Median time to acknowledgment.
  • Median time to written response.
  • Aggregate resolution outcomes.
  • Net satisfaction score from post-care surveys, if collected.

H. Privacy and Security

  • Number of HIPAA reportable breaches affecting Teledris Health patients in the period.
  • Number of non-reportable security incidents reviewed.
  • Status of annual HIPAA risk assessment and SOC 2 attestation, when applicable.

I. Patient Rights Exercised

  • Number of CCPA requests received by category.
  • Number granted, partially granted, or denied.
  • Median response time.
  • Number of HIPAA access, amendment, accounting, and restriction requests received and processed.

J. Workforce and Clinician Network

  • Number of licensed clinicians actively providing care.
  • States covered.
  • Median clinician panel size.
  • Whether clinicians are W-2 employees of the Affiliated Provider, contractors, or both.

IV. Partner and Structural Disclosure

We believe patients should know who actually delivers their care.

A. Care Delivery Structure

Teledris Health, operated by KavrenTech, is the management services entity. Clinical care is delivered by [Affiliated Professional Entity Name], a professional medical entity whose clinicians are licensed in the states we serve.

B. Pharmacy Partners

The compounding pharmacies dispensing medication to Teledris Health patients are [Pharmacy Partner List — to be confirmed]. Each is licensed in the relevant states and follows USP <797> sterile compounding standards.

C. Technology Partners

Patient-facing and clinical infrastructure partners include our electronic health record, secure messaging, telephony, identity verification, payment processing, and analytics providers.

D. Independent Oversight

We commit to engaging an independent auditor for SOC 2 Type II attestation no later than 24 months after launch. We do not currently have a formal patient advisory board and are evaluating one for year two.

V. Methodology Notes

  • Reporting periods run January 1 to June 30 and July 1 to December 31.
  • Patient counts are de-duplicated by patient.
  • Time-to metrics are computed from server timestamps in business hours unless noted.
  • Test accounts, internal QA accounts, and fraudulent accounts are excluded.
  • Counts under five are reported as <5 to protect privacy.
  • If we discover an error in a prior report, we will publish a correction in the next edition with a clear note.

VI. Commitments

  1. Publish the first quantitative report at approximately month 6 post-launch.
  2. Publish on a semi-annual cadence going forward, within 60 days of the period close.
  3. Maintain consistent definitions and methodology across reports.
  4. Publish the report as a standalone document on our website, freely accessible without registration.
  5. Disclose any material change to scope or methodology in the report itself.
  6. Do not stop publishing because the numbers are unflattering.
