Patient Bill of Rights

April 2026

I. Our Commitment to You

Teledris Health, operated by KavrenTech, is committed to providing care that respects your dignity, your choices, and your privacy. This Patient Bill of Rights describes the rights every Teledris Health patient is entitled to, and our responsibilities to you.

II. Your Rights

A. Right to Dignity, Respect, and Non-Discrimination

You have the right to receive care that is courteous and respectful, regardless of your race, color, ethnicity, national origin, religion, age, sex, gender identity, sexual orientation, marital status, disability, veteran status, primary language, or source of payment.

B. Right to Privacy and Confidentiality

You have the right to expect that information about you will be kept private and used only for the purposes described in our Privacy Policy and Notice of Privacy Practices. Your right to privacy includes:

  • Confidentiality of your medical record under HIPAA.
  • Reasonable measures to secure communications about your care.
  • The right to ask how your information may be used or shared, and to limit certain uses where the law allows.

D. Right to Refuse or Withdraw From Treatment

You have the right to refuse any treatment or to withdraw from treatment at any time. Your clinician will explain any clinical or medical consequences of refusing or stopping treatment.

E. Right to a Second Opinion

You have the right to seek a second opinion from another clinician, inside or outside of Teledris Health, at any time.

F. Right to Access Your Medical Records

You have the right to inspect and obtain a copy of your medical record. Requests are processed through our Privacy Officer.

  • We will respond within 30 days of a written request, with one 30-day extension permitted under HIPAA, with notice to you.
  • Where state law requires a faster response, we comply with the state-law timeline.
  • We may charge a reasonable, cost-based fee for copies, and we will tell you the fee in advance.

G. Right to Communicate in Your Primary Language

We provide our services in English and Spanish at launch. Where you require a language we do not currently offer, we will work with you to identify reasonable accommodations.

H. Right to Know Your Provider's Credentials

You have the right to know the name, title, license, and state of licensure of any clinician providing your care. This information is provided when you are matched with a clinician and is also available on request through your account or by contacting support@teledrishealth.com.

I. Right to Transparent Pricing

You have the right to know the cost of any service or medication before you are charged. We commit to clear disclosure at the point of purchase and itemized charge records on request.

J. Right to Continuity of Care

You have the right to continuity of care to the extent we can provide it. If your clinician becomes unavailable or if Teledris Health stops offering care in your state, we will notify you and support a transition.

K. Right to File a Grievance

You have the right to file a grievance about your care, our staff, or our services without fear of retaliation. We commit to:

  • Acknowledging your grievance within 2 business days.
  • Providing a written response within 30 days.
  • Conducting a fair review.

To file a grievance, email compliance@teledrishealth.com with the subject line "Grievance" or write to the Compliance Officer at 7800 SW 87th Ave, Miami, FL 33155.

You may also file complaints with:

  • Your state medical board for concerns about a clinician’s care or conduct.
  • U.S. Department of Health and Human Services, Office for Civil Rights — https://www.hhs.gov/ocr
  • Federal Trade Commission — https://reportfraud.ftc.gov
  • Your state attorney general’s office for state consumer protection issues.

L. Right to Information About Clinical Research

We do not currently conduct clinical research with our patients. If that ever changed, you would have the right to be informed and to choose whether to participate.

M. Right to Information About Advance Directives

Although Teledris Health does not provide acute or end-of-life care, you have the right to make and document advance directives under your state's law.

N. Right to Be Free From Abuse, Exploitation, or Harassment

You have the right to receive care free from abuse, exploitation, harassment, or coercion. Any concern of this nature should be reported immediately to compliance@teledrishealth.com.

O. Right to Personal Safety

You have the right to expect reasonable steps from us to protect your personal information and to handle your medication safely. If you believe your safety has been compromised, contact us immediately.

III. Your Responsibilities

Care is a partnership. To support your right to safe, effective care, you agree to:

  • Provide accurate, complete, and current information about your health.
  • Disclose all medications, supplements, allergies, and conditions.
  • Follow your treatment plan and report side effects promptly.
