By acknowledging this document electronically and proceeding with care through Teledris Health, you confirm that you have read and understood the information below, that your questions have been or will be answered to your satisfaction, and that you consent to receive telehealth services and, if prescribed, compounded medications under the conditions described.
I. About This Consent
This Informed Consent document describes the nature, benefits, risks, and limitations of receiving healthcare through Teledris Health's telehealth platform, including the use of compounded GLP-1 receptor agonist medications when clinically appropriate. It also describes your rights and responsibilities as a patient.
Teledris Health, operated by KavrenTech, is located at 7800 SW 87th Ave, Miami, FL 33155. Care is delivered by licensed clinicians employed by or contracted through our affiliated professional entity.
This document does not replace your relationship with your treating clinician. You may ask questions at any time, and you may withdraw your consent at any time without penalty for future care, although you may remain responsible for charges already incurred.
II. What Telehealth Is
Telehealth is the delivery of health care services using interactive electronic communication, including secure messaging, written intake forms, video, audio, photos, and data transmission, to evaluate, diagnose, and treat patients without an in-person visit.
Teledris Health uses both:
- Asynchronous, or store-and-forward, telehealth, in which you submit information through a secure platform and a clinician reviews it later.
- Synchronous telehealth, in which you and a clinician communicate in real time by video or telephone.
Most Teledris Health visits begin asynchronously. Live audio or video visits may be required by your state of residence, by your clinical situation, or at the clinician's discretion.
III. Benefits of Telehealth
- Convenient access to licensed clinicians without travel.
- Care from your home or another private location.
- Faster scheduling than many traditional in-person practices.
- Continuity of care through ongoing secure messaging.
- Bilingual support in English and Spanish.
IV. Risks and Limitations of Telehealth
Telehealth has limitations that do not exist in an in-person visit. By consenting to telehealth, you acknowledge that you understand and accept these limitations:
- No physical examination. Some conditions cannot be diagnosed or treated safely without an in-person examination.
- Information accuracy. The accuracy of any diagnosis or treatment plan depends on the accuracy and completeness of the information you provide.
- Technology failures. Internet outages, dropped calls, software errors, or device malfunction may interrupt or delay care.
- Privacy and security. We use industry-standard encryption and security measures, but no system is completely secure.
- Recording, transmission, and storage of PHI by Teledris Health and authorized vendors. We do not record video visits without your separate consent.
- Limited diagnostic scope. Your clinician may identify a condition outside the scope of services Teledris Health provides and refer you elsewhere.
- Continuity with other providers. Telehealth care may not be sufficient by itself, and we strongly encourage you to maintain a relationship with a primary care provider.
V. Emergency Limitations
TELEDRIS HEALTH IS NOT AN EMERGENCY SERVICE.
If you are experiencing a medical emergency, including chest pain, difficulty breathing, severe abdominal pain, signs of stroke, severe allergic reaction, suicidal ideation, or any life-threatening symptom, call 911 or go to your nearest emergency room immediately.
If you are in crisis, you may also contact the 988 Suicide and Crisis Lifeline by calling or texting 988.
VI. Establishment of the Provider–Patient Relationship
By submitting an intake and being accepted for care, you and your treating clinician establish a provider–patient relationship through telehealth, consistent with the laws of your state of residence.
Your clinician may decline to establish a provider–patient relationship if your clinical situation is outside the scope of telehealth practice or outside Teledris Health's services. If treatment is declined, you may be issued a partial or full refund of your consultation fee in accordance with our Refund Policy.
VII. About Compounded GLP-1 Medications
Teledris Health may prescribe compounded GLP-1 receptor agonist medications, such as compounded semaglutide and compounded tirzepatide. The following disclosures apply if you receive a compounded medication:
A. What "Compounded" Means
A compounded medication is a medication prepared by a licensed compounding pharmacy specifically for an individual patient based on a prescription from a licensed clinician. Compounded medications are made from active pharmaceutical ingredients combined and finished by the pharmacy.
B. Compounded Medications Are Not the Same as Branded Medications
Compounded semaglutide and compounded tirzepatide are not the same as Wegovy, Ozempic, Zepbound, or Mounjaro. Branded medications are FDA-approved products manufactured under strict FDA oversight.
Compounded medications dispensed through Teledris Health partners are prepared by state-licensed 503A compounding pharmacies, or where applicable 503B outsourcing facilities. Compounded products are not subject to FDA approval of safety, efficacy, or manufacturing standards in the same manner as branded drugs.
C. Why Compounded Medications May Be Available
Compounded GLP-1 medications may be lawfully dispensed when clinically necessary for an individual patient, and historically have been more widely available during periods when branded medications were on the FDA shortage list. The shortage status of these medications can change.
D. Sources and Quality
Our partner compounding pharmacies are state-licensed and follow USP <797> compounding standards. We obtain Certificates of Analysis for active pharmaceutical ingredients from FDA-registered facilities. Despite these safeguards, you acknowledge that compounded medications carry inherent variability that does not exist with branded products.
VIII. GLP-1 Treatment: Risks, Side Effects, and Warnings
GLP-1 receptor agonist medications are powerful drugs with significant clinical effects. Your clinician will discuss your specific situation with you. The following disclosures apply to the class of GLP-1 medications and to compounded versions of those medications.
A. Boxed Warning — Risk of Thyroid C-Cell Tumors
Branded GLP-1 receptor agonists such as semaglutide and tirzepatide carry a boxed warning for risk of thyroid C-cell tumors based on rodent studies. The same warning applies, as a precaution, to compounded versions of these medications.
You must not take a GLP-1 medication if you or any member of your family has a history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2. You must report any neck mass, hoarseness, persistent difficulty swallowing, or shortness of breath to your clinician promptly.
B. Pancreatitis
GLP-1 medications have been associated with cases of acute pancreatitis, including fatal and non-fatal hemorrhagic and necrotizing pancreatitis. Stop the medication and contact your clinician or seek emergency care for severe abdominal pain that may radiate to the back, with or without vomiting.
C. Gallbladder Disease
GLP-1 medications have been associated with cholelithiasis and cholecystitis. Report severe right-upper-quadrant abdominal pain, fever, or jaundice promptly.
D. Hypoglycemia
The risk of hypoglycemia is increased when GLP-1 medications are used together with insulin or insulin secretagogues such as sulfonylureas. If you take any of these medications, dose adjustments may be required, and you must disclose all current medications during intake and at every follow-up.
E. Acute Kidney Injury
GLP-1 medications can cause nausea, vomiting, and diarrhea, which can result in dehydration and acute kidney injury, particularly in patients with pre-existing kidney disease. Maintain adequate fluid intake and report symptoms of dehydration or decreased urine output.
F. Hypersensitivity Reactions
Serious hypersensitivity reactions, including anaphylaxis and angioedema, have been reported. Stop the medication and seek emergency care for swelling of the face, lips, or tongue, difficulty breathing, or hives.
G. Diabetic Retinopathy Complications
In patients with type 2 diabetes and a history of diabetic retinopathy, rapid improvement in glucose control with GLP-1 medications has been associated with worsening of retinopathy. Inform your clinician if you have a history of diabetic eye disease.
H. Gastrointestinal Effects
The most common side effects are gastrointestinal: nausea, vomiting, diarrhea, constipation, abdominal pain, indigestion, decreased appetite, and reflux. Most are mild to moderate and improve with time and dose adjustment. Severe or persistent symptoms should be reported.
I. Gastroparesis and Ileus
Cases of gastroparesis and ileus have been reported. If you have a history of severe gastroparesis, GLP-1 therapy may not be appropriate.
J. Suicidal Ideation and Mood Changes
Post-marketing reports have noted suicidal ideation in some patients on GLP-1 medications. The relationship is unclear. Report any new or worsening depression, mood changes, or thoughts of self-harm to your clinician immediately and seek emergency care if you are in crisis.
K. Pregnancy, Breastfeeding, and Fertility
GLP-1 medications must not be used during pregnancy. If you are pregnant, planning to become pregnant, or breastfeeding, you must disclose this and you should not begin or should stop GLP-1 therapy.
GLP-1 medications may reduce the effectiveness of oral contraceptives during dose escalation. Use a non-oral or backup contraceptive method. GLP-1 therapy should be discontinued at least two months before a planned pregnancy.
L. Anesthesia and Surgery
Because of the risk of delayed gastric emptying, your clinician may instruct you to hold doses before scheduled procedures requiring anesthesia. Always inform anesthesia and surgical providers that you are taking a GLP-1 medication.
M. Weight Regain After Stopping
Discontinuation of GLP-1 therapy is commonly followed by partial weight regain. Long-term outcomes depend on continued lifestyle change and, in some cases, continued medication.
N. Off-Label Use
Some GLP-1 prescriptions may be considered off-label for your specific situation. Off-label prescribing is legal and common in U.S. medicine; it means a clinician is using clinical judgment to prescribe a medication for a use not specifically listed on the FDA-approved label of the branded equivalent.
IX. Monitoring and Follow-Up
You agree to participate in monitoring and follow-up as directed by your clinician. This typically includes:
- Periodic check-ins through the Teledris Health platform.
- Reporting your weight on the cadence requested.
- Reporting side effects, new symptoms, and any new medications.
- Completing requested laboratory tests if ordered.
- Notifying us before any planned surgery or anesthesia.
- Notifying us if you become pregnant or are planning pregnancy.
Failure to participate in monitoring may result in pausing or stopping your treatment.
X. Patient Responsibilities
By accepting care through Teledris Health, you agree to:
- Provide accurate, complete, and current information at intake and at every follow-up.
- Notify us promptly of any new diagnosis, hospitalization, surgery, or new medication.
- Follow the dosing instructions provided by your clinician and the partner pharmacy.
- Store medication as directed and never share it with another person.
- Properly dispose of needles and unused medication.
- Use the Teledris Health platform for non-emergency communication only.
- Pay for services and medications as agreed.
XI. State-Specific Provisions
The following addenda apply if you are a resident of, or are physically located in, the listed state at the time of your visit. State-specific telehealth law may impose additional requirements.
A. California
California law requires that prior to delivering health care via telehealth, your provider obtains your verbal or written consent. Your electronic acknowledgment of this Consent satisfies this requirement.
B. Texas
Texas law may require an in-person follow-up or specific documentation in certain circumstances. Your clinician will inform you if this applies to your care.
C. New York
New York patients have additional rights under the New York Patient Bill of Rights. Please review the Teledris Health Patient Bill of Rights.
D. Florida
Florida-licensed clinicians providing care to Florida residents follow the Florida Telehealth Act. Out-of-state Florida-registered telehealth providers comply with Section 456.47, Florida Statutes.
E. Other States
State-specific addenda will be added as Teledris Health expands its operating footprint. The full list of states in which Teledris Health is currently available is published on our website.
XII. Right to Withdraw Consent
You have the right to withdraw your consent to telehealth or to a particular treatment at any time by notifying us in writing at support@teledrishealth.com or through the platform.
Withdrawal of consent will not affect care already provided, and you may remain responsible for fees already incurred. Withdrawal will end any subscription on its next billing date in accordance with our Refund Policy.

By acknowledging this Consent electronically and proceeding to care, you confirm that:
- You have read and understood this Consent.
- You have had the opportunity to ask questions, and any questions have been or will be answered to your satisfaction by your clinician.
- You voluntarily agree to receive care through telehealth and, if prescribed, to use compounded GLP-1 medication.
- You have disclosed accurate and complete medical information.
- You understand the differences between compounded and branded medications.
- You understand the risks, including the boxed warning regarding thyroid C-cell tumors.
- You agree to participate in monitoring and follow-up.
- You understand your right to withdraw consent at any time.
